Book about woman falling in love with the same man three times. The man has changed appearance through plastic surgery.

Many years ago, i read a captivating book about a woman who ends up falling in love madly with the same man 3 times. The first time she meets him he's an artist (i think). She ends up troubled by this relationship and disappears from his life. Years later she meets a race car driver, she falls madly in love again, only to realize later that it was the same man as her first love, but he has completely changed appearance and behavior. She disappears again. Years later again, she falls in love with a sailor, and again she later realizes it was her first lover who has again changed appearance and behavior to make her fall in love. This is all i can remember from the plot. I remember it being thrilling and unexpected. I don't know the title, author or book cover. If someone can help me i would greatly appreciate it because I've been trying to find it for years now.
